Abstract: At 2:40 p.m. on July 19, 2019, Employee #1 measured out a small sample of product to be shipped out. After collecting the sample, the employee attempted to reclose the bag. He folded the top of the bag over and used a Newlong Portable Bag Closer, Model NP-7A, and Serial Number 100393, to sew the bag shut. The bag closer became stuck as it sewed into the bag. As the employee attempted to unjam the bag closer by lifting the bag up so the weight of bag would help clear the jam, his finger entered an unguarded pulley with a opening, causing the employee's left index finger tip to be amputated.
