Abstract: At 8:45 p.m. on May 4, 2018, Employee #1, employed by a demolition construction company, was working at a residential construction site. He was standing on the roof top of a truck and pulling a dump box cover when he lost balance and fell. Emergency services transported the employee to the hospital, where he was admitted and treated for a fractured nose and lacerations to his lips.
