Abstract: At 1:00 p.m. on November 18, 2017, an employee was in the shop working on a rig's malfunctioning side-loader cylinder. The employee tested the sensor by touching it with his screwdriver from the backside of the carriage which placed his hand and arm under the elevated cart. The cylinder activated and trapped the employee's hand and arm. A coworker freed the employee and the employee was taken to medical faciltiy for treatment. The employee suffered injuries to his left hand, fractured his wrist and had a severe laceration to his forearm. The employee was hospitalized for a two week period and the employee elected to have his left forearm amputated.
