Abstract: At 4:50 p.m. on February 26, 2018, Employee #1 was standing on the lower rear cross beam, manually cleaning and removing any visible remnants of cookie dough from the edge of the case of an APV Baker Perkins dough logger. The machine was off, but while cleaning, the drum/dough chunker was activated. This created a pinch point between the stationary case and the rotating dough chunker drum. His right index, middle, and ring fingers were caught in the pinch/nip point, causing partial amputation of the fingers. Employee #1 was admitted on to the hospital for treatment.
