Abstract: At 9:45 a.m. on February 1, 2018, Employee #1, employed by a construction company, was engaged in concrete form work at a multiemployer building construction site. He slipped, tripped, and fell, a fall height of approximately 46 inches. Employee #1 fell onto a Number 5, .625-inch rebar, which impaled his left thigh. Emergency services were called, and Employee #1 was transported to the hospital, admitted, and treated for the impalement.
