Abstract: At 6:00 a.m. on February 9, 2018, an employee had responded to a service call for a tire replacement for the passenger side rear tire on axle #5 for a trucking semi-tanker on I-294 northbound during a heavy snowfall. The employee was caught under the passenger rear side bumper of the truck, estimated gross weight of 75,680 lbs, when the supporting jack failed. The employee s chest area was crushed by the truck and was transported to a hospital where he died from his injuries.
