Abstract: At 1:51 p.m. on December 15, 2017, an employee was working with three coworkers cutting in a key way and preparing to install forms for retaining wall footings when a section of the wall and the soil behind it collapsed into the trench. The employee was crushed and covered by a concrete retaining wall and soil up to his chest, killing him. Coworkers #1 and #3 escaped from the trench without injuries.
