Abstract: At 11:00 a.m. on November 28, 2017, Employee #1 was tasked to repair the tailgate door of a garbage truck, located at the rear section of the truck. The tailgate door of the Mack front loader garbage truck, Vehicle Identification Number: 1M2K195C54M025743, was approximately 95 feet wide by 100 feet high. Employee #1 placed and rested the tailgate door on top of the mast of a Toyota powered industrial truck (PIT), Serial Number: D738330. The top of the mast was approximately 93 feet above the ground. The tailgate door weighed approximately 600 pounds. Employee #1 was located between the tailgate door and the garbage truck hopper. The hopper was the section of the truck where debris was dumped when collecting garbage. The hopper opening was approximately 92 feet wide by 82 feet high. The hopper opening was approximately 52 feet above the ground. A mechanic was located on the PIT. Another coworker was replacing parts in the cab of a Ford aerial boom lift truck, Vehicle Identification Number: 1FDAF56FXXED11844. The truck was positioned at the right rear of the garbage truck. The mechanic needed to use the PIT to move equipment inside the third bay of the garage. The mechanic raised the mast and backed the PIT. The tailgate fell and caught Employee #1's head between the tailgate door and the lower section of the hopper. Employee #1 received skull fractures and was killed.
