Abstract: At approximately 9:00 a.m. on September 4, 1992, Employee #1 and two coworkers were attempting to lift a stud wall measuring 172 in. long by 144 in. wide. The wall slid on the wet plywood subfloor and hit a previously erected wall. This collision caused the standing wall to fall back onto the wall that the three workers were raising. Unable to hold both walls, the two coworkers at the ends were knocked out from the walls. Employee #1, who was in the middle, was struck down and both walls fell on his chest, crushing him. Employee #1 was pronounced dead at the hospital shortly after arrival. He suffered a crushed chest, with bone fragments that penetrated his heart, and internal bleeding. Causal factors included misjudgment of a hazardous situation. No safety violations were noted.
