Investigation Summary
At approximately 11:45 a.m. on March 14, 2021, Employee #1, a spotter, was assisting a forklift operator by identifying a location for merchandise placement. While the spotter was observing the area, the forklift operator, who was looking in the opposite direction, moved the forklift and accidentally crushed the spotter's left foot. The employee sustained a crush injury to his left foot and was hospitalized for treatment.
