Employee's fingers are crushed in power press during setup,

At 2:15 p.m. on January 7, 2021, Employee #1 was performing setup operations on a mechanical power press. While attempting to remove a part from the press, the supervisor cycled the machine as Employee #1's hand was inside the die area. The incident resulted in the crushing of three fingers: the middle, ring, and tip of the small finger on the employee's right hand. Due to the extent of the injuries, medical amputation of the affected fingers was required. Employee #1 was transported to the hospital, admitted, and treated for the injuries.
