Investigation Summary
At 8:15 a.m. on May 13, 2020, an employee was working as a counselor/manager for a substance case. The employee was attending a counseling session with a resident client on April 30,2020 but did not keep adequate social distancing while attending a client on the 2nd floor. The employee was counseling the client while approximately four feet away from the client within the breathing zone for at least 20 minutes without face covering or proper PPE. The client was subsequently tested for COVID-19 on 5/1/20 and test results were confirmed to be positive on 5/2/20. The employee developed COVID-19 like symptoms on 5/6/2020, reported symptoms and requested a sick leave on 5/7/2020. The employee was hospitalized from 5/13/2020 through 5/15/2020 and was back at work on 5/29/2020 COVID-19 virus that resulted in pneumonia.
