Investigation Summary
At 11:30 a.m. on July 1, 2020, Employee #1, employed by a construction company, was engaged in the construction of a building at a farm. He was working from the roof's wood trusses and installing 2 by 4 wood purlins when he fell, a fall height of 20 to 22 feet, and struck the concrete floor. Employee #1 was not using any form of fall protection. He was transported to the hospital, where he began receiving treatment for head and back injuries. Employee #1 died at the hospital.
