Employee amputates middle finger when contacting rotating bl

At 4:20 a.m. on March 27, 2020, Employee # 1 was unclogging rice in R-Valve #17 in Mill L that was not locked out/ tagged out. Employee # 1 reached into R-Valve #17 to retrieve an iron rod used for unclogging the filters. He suffered an amputation of the middle-left hand finger from the rotating blades in the machine. The employee was treated without hospitalization.
