Investigation Summary
At 8:00 p.m. on January 16, 2020, Employee #1 and Employee #2 were riding in the back of a flatbed truck moving metal frame and highway caution signs. Two coworkers rode in the inside of the truck cab and were driving. While the truck was moving, one of the highway caution signs flew up and struck Employee #1 and Employee #2. Employee #1 fell from the moving truck and struck his head on the highway road, resulting in fractures to his head and was killed.
