Investigation Summary
At 9:11 a.m. on December 31, 2019, an employee was manually rolling pipe onto a rack to be picked up by a forklift. A pipe that was too short for the rack fell off. Just before falling off, the employee stuck part of his hand into the pipe. His hand was caught between the falling pipe and the rack. The employee sustained unspecified injuries to his fingers.
