Employee is pinned after forklift pushed containers

At approximately 6:00 a.m. on August 20, 2019, an employee adjusted the mast on a forklift. A coworker was unloading a 2 stacked high, HDU metal framed container from a trailer. The HDU has a dimension of approximately 4ft by 4ft by 4ft. MH #1 was then putting the containers in a narrow aisle, and at the other end, the employee was putting the containers in their specific location. For an unknown reason, the employee got off the forklift, and stood between the containers and forklift mast to adjust the forks. At the same time, the coworker pushed the line of containers forward and pinned the employee between the forklift mast and HDU containers. The employee suffered lung contusions, a lacerated spleen, and fractured ribs and was hospitalized for treatment.
