Investigation Summary
At 9:15 p.m. on April 2, 2019, an employee was operating a shop made bell press to conduct de-mil operations on a 120mm round and reached in to adjust the round on the stand. The employee caught his right index finger between the press and round, amputating his fingertip. The employee was hospitalized.
