Investigation Summary
At 10:00 a.m. on February 21, 2019, an employee was working with a Parkinson Technologies, Inc. extruder machine (Model Number 004032-01). When the roll reached the correct size, he began to remove the roll to replace it. The employee placed his hands on the collar when it cracked and his left hand was pulled into the crack. The employee amputated his hand. He went to a clinic and then to Western Hand Clinic for treatment, but hospitalization was not required.
