Investigation Summary
At 8:45 a.m. on March 1, 2019, an employee was operating a converting line machine. The employee was changing Teflon tape at the bottom of the heat bar, when he hit the emergency shut down rope causing the machine to activate moving into the closed position, catching the employee's finger under the heat bar. This resulted in an amputation of the left index finger to knuckle.
