Investigation Summary
At 12:33 a.m. on December 11, 2018, an employee was setting up the Repak 7 machine used in packaging tamales when the plastic film she was installing jammed. The employee reached under the machine guard and amputated several fingers when the equipment cycled itself and caught them. The employee was hospitalized.
