Investigation Summary
At 9:00 a.m. on Janurary 17, 2019, Employee #1 was cutting sheets of very thin (1/32 of an inch) laminate using a table saw equipped with a 10-inch circular blade. As the table saw's fence was set to the right of the blade, the employee used his right hand to push the laminate sheet into the saw blade as his left hand moved with the laminate sheet to keep it pressed against the table saw fence. As the employee guided the laminate sheet into the saw blade, his left hand index, middle, and ring fingers were struck by the saw blade which caused lacerations to left hand index and middle fingers and the amputation of the ring finger. The employee was hospitalized.
