Employee is crushed and killed by falling forklift

At 2:00 p.m. on January10, 2019, an employee was servicing/ changing the oil on a Caterpillar forklift Model Number: GC40KS, Serial Number: AT87A00203. Coworkers lifted the forklift with a "sky track" so that the employee could access the oil filter. The forklift slipped from the "sky track" and fell inclined on the two wheels (on one side). The employee was harnassed on the side of the forklift. The forklift fell completely on its side and crushes the employee. The employee was killed from a skull fracture.
