Investigation Summary
At 9:45 a.m. on November 15, 2018, an employee was chipping out dried concrete from a hopper with a jackhammer in the dry cast department. The employee was inside the concrete hopper breaking out the concrete when a portion of concrete fell, striking the employee in the foot. The employee was transported to a local hospital, where he had multiple toes amputated.
