Employee amputates two fingers in meat mixer

At 5:00 a.m. on November 7, 2018, Employee #1 was removing v-mags of ground meat from the mixer. The safety interlocks on the machine guarding failed, and the machine turned on. Two fingers on his right hand were amputated by the left auger on the left side of the discharge door. The employee was hospitalized for treatment.
