Investigation Summary
At 1:50 p.m. on June 27, 2018, Employee #1 was removing resin from the door of a Banbury machine while it was in operation. The door cycles open and closed during production at intervals. The employee was using a scraper that was approximately 4 feet long to remove the resin. His arm was pulled into the machine when the door closed, and he incurred multiple fractures and second degree burns. The employee was hospitalized to treat the fractures and second degree burns.
