Investigation Summary
At 7:30 p.m. on May 3, 2018, an employee was trouble shooting a drive belt on a feeder. The employee removed the guard over the drive belt and touched the belt to check the tension. The drive belt spun and caught the employee's right index finger, resulting in a partial amputation.
