Investigation Summary
At 1:30 p.m. on April 13, 2018, Employee #1, employed by a drilling construction company, was operating an Akkerman Guided Boring Machine Model 308A with jacking 10-inch diameter steel casing. As he was removing steel wedges used to connect casing, he bumped and activated the thrust control lever. His head was caught between the machine's push plate and front frame. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for multiple facial and jaw fractures and crushing injuries.
