Investigation Summary
At 4:15 p.m. on August 7, 2017, an employee was operating a PIT in a trailer. The employee fell out of the trailer when the coworker mistakenly drove the truck forward. The employee was struck by the PIT's overhead guard plate and received a crushing injury to the lower right leg, resulting in amputation.
