Investigation Summary
At 8:15 a.m. on June 5, 2017, an employee was assisting in the delivery and staging of roof shingles at a residential construction site. While the employee was located the roof, standing along the ridgeline, a coworker tossed him a bundle of shingles. The employee lost his balance while trying to juggle the load and stepped backwards, falling between 12 to 15 feet to the grass below. The employee sustained a fractured pelvis, which he was hospitalized and received treatment. The incident investigation revealed that no fall protection was in use at the time of the incident.
