Investigation Summary
At 12:00 p.m. on May 8, 2017, an employee was operating a straw wrapping machine when a jam occurred. As the employee placed his left hand under a guard inside the machine's hopper to clear a jam, his index finger became caught by its roller belt. As a result, the tip of his left index finger was amputated. The employee was hospitalized and received treatment for his injury.
