Employee Receives Severe Burns When 5-gallon Tank Catches Fi

At approximately 4:45 p.m. on May 23, 2017, Employee #1 was filling a customers 5-gallon propane tank with propane. The connection from the main tank to the 5-gallon tank was cross threaded. When Employee #1 tried to repair it, the connection popped off the tank and created a vapor cloud that ignited and created a fire and explosion. Employee #1 suffered second and third degree burns on her arms, chest and upper legs in the fire and explosion. Employee #1 was transported to a medical center, where she underwent burn treatment for her serious burns and was then hospitalized.
