Investigation Summary
On May 23, 2017, Employee #1 was on the passenger side of a trailer, unhooking and removing the other two-inch ratchet strap from the slab lift clamp. The slab lift (forklift) clamp, which stood approximately 6 ft high and 5 ft wide was strapped down on a utility trailer, that had 18 in. high perimeter guardrails. A coworker had unhooked one of the two-inch ratchet straps from the slab lift on the driver side of the trailer. A second coworker was operating the forklift on the driver's side of the trailer. As the second coworker was getting the forklift into position, it appears that one of the forks of the forklift may have hit the top right corner of the clamp, causing the clamp to roll over off of the trailer and strike Employee #1's left leg. The falling slab lift clamp fractured his left leg above the ankle. Employee #1 was transported to a medical center, where he received treatment on the leg fracture and was then hospitalized.
