Investigation Summary

Investigation Nr: 95808.015
Event: 05/17/2017
Tip of employee's thumb is amputated during servicing of RAP

At 6:45 a.m. on May 17, 2017, an employee was investigating the malfunction of a RAP bin motor, which the motor/shaft was binding. As the employee grabbed the belt of the motor and worked to free the stuck parts, the motor came on. The motor parts, including the belt, began moving and contacted the tip of one of the employee's thumbs. The tip of his thumb was amputated as a result of becoming tangled between the belt and pulley of the motor.
