Employee's fingers are amputated with use of hydraulic press

At 9:45 a.m. on May 16, 2017, an employee was using a Wysong press brake machine (model number: MTH175-120) to custom shape flat metal to be used with conveyor belts. The employee was feeding a piece of 3 inch wide by 63.5 inch long stainless steel stock metal into the press brake when the fingers on both hands, which were gloved, were crushed by the hydraulic blade. The electronic light curtain, and two hand button safety devices were not engaged at the time of the incident. The tips of his index and middle fingers on the left hand, and the tips of his middle and ring fingers on the right hand were amputated.
