Employee amputates finger in a hydraulic crimper

At 1:30 a.m. on May 4, 2017, Employee #1, a maintenance trainee, was observing the maintenance leader troubleshooting the hydraulic system of the bending, cripping, and punch (BPH) equipment. The maintenance leader had instructed the supervisor to turn on the power to the BPH machine and did not observe that the employee had inserted his finger in the lower part of the hydraulic system to clean out the debris. The hydraulic crimper pressed down on the employee's wedding ring finger, which resulted in a partial amputation of the finger.
