At 7:00 p.m. on May 25, 2017, an employee walked on an angle in front of an approaching powered industrial vehicle (PIV) operated by a coworker. The coworker did not see the employee approaching and struck him. The employee's leg was run over by the PIV and resulted in a crushed leg that later required amputation.
