Investigation Summary
At approximately 6:35 a.m. on April 7, 2017, Employee #1 was operating a 5 ft panel rolling machine. A wire had come off the center-straightening roller of the machine. He used a pair of pliers to grab the wire and try to pull it back onto the roller. The machine cycled and pulled the wire 8 in. back into the machine. The pliers were pulled in-between the frames of the straightening mechanism. His left ring finger was caught between the frame and the handle of the pliers, causing the tip of the ring finger being amputated. Employee #1 was transported to a medical center, where he received treatment and was then released.
