Investigation Summary
At approximately 1:55 p.m. on July 14, 2016, Employee #1 and two coworkers were stacking used oak barrels, weighing approximately 110 pounds each. They were stacking on the sixth tier making a seventh, approximately 12 foot high. Employee #1 was moving a barrel on the edge when he slipped and fell landing on the ground. Two barrels also fell and struck Employee #1 on his head and he was killed.
