Investigation Summary
At 9:15 a.m. on September 12, 2014, an employee was working from a ladder to install shelves on an industrial racking system. For an unknown reason, the employee climbed onto the shelf being installed. The shelf was approximately 8 feet above a concrete floor. While he was on the shelf, it gave way and the employee fell head first to the ground. He was killed.
