Employee Is Crushed While Adjusting Production Parts

At 4:00 p.m. on June 10, 2014, an employee was operating a bumper fascia sequencing machine, which presses bumper parts together. The employee had placed parts on the lower table within the machine to be assembled and cycled it. The employee stopped the machine mid-cycle when he noticed that the parts were not properly aligned and bent into the machine to realign them. The machine cycled itself while the employee was in it and crushed his head between the pneumatic ram and the bumper parts on the lower able. The employee was transported to the hospital and died the next day from his injuries.
