Investigation Summary
At approximately 9:15 a.m. on October 11, 2013, Employee #1 was tasked with installing welding studs on a second story deck. He first had to measure the decking area to place the welding studs. There was an unprotected floor opening (20 ft by 5 ft) to the lower level on the deck. As Employee #1 was measuring the deck, he possibly walked backwards and fell through the opening. Employee #1 fell from the deck and sustained unspecified injuries when he landed on the lower level that killed him.
