Employee is found dead on floor with skull fracture

At approximately 8:15 p.m. on August 20, 2013, Employee #1, a second shift employee, was working alone in an aisle where oil drums were located. Part of his duties included drawing lubricating oil from the 55-gallon drums for various machines. Employee #1 was tall enough to comfortably reach any valve on the drums without the need to stand on a platform. A coworker heard a noise and found Employee #1 lying in the middle of the aisle with a skull fracture. Employee #1 died.
