Employee is crushed by falling steel plate, later dies

At approximately 6:00 p.m. on February 6, 2013, Employee #1 had removed a steel framework that was used to contain radiation detectors. The north and south side framework walls were removed and hauled away, however the steel plate, back wall, was not removed. Employee #1 was working alone when the steel plate, measuring 95-in. by 86-in. and weighing more than 300 pounds, fell and pinned Employee #1 against the ground. Employee #1 suffered a broken right ankle, three cracked ribs, and a fracture of his left collarbone. Employee #1 underwent surgery on February 8, 2013 and ultimately died from cardiac arrest.
