Investigation Summary
At approximately 6:00 p.m. on February 8, 2013, Employee #1 was raising a metal storage rack with a forklift. He exited the forklift to steady or adjust the rack supported by the forks. He stood under the rack when it fell, crushing him. Employee #1 killed.
