Investigation Summary
At approximately 2:30 p.m. on October 21, 2012, Employee #1 was tasked to retrieve lights and was using a scissor lift to help transport the light fixtures and parts. He was redirected by his supervisor to fix a light fixture that was not working. Employee #1 did not have gloves, rubber mats or any other type of ppe. The employer did not provide additional lighting and had the breakers on and the power live to the lights. Employee #1 was not a qualified electrician and when he raised the scissor lift to work on the light, he grabbed a live 277 volt wire. He made contact with the live wire and was leaning on and holding the guardrail of the non-electrically insulated scissor lift. Employee #1 was electrocuted.
