Investigation Summary
At approximately 4:00 p.m. on June 4, 2012, an employee was carrying a sheet of permanent metal decking that was to be installed over a hole on a highway that was under construction. The employee and a coworker were installing permanent metal decking over a drainage opening on an elevated section of highway. The rectangular drainage opening measured approximately 8-ft. in length by 6-ft. across. Two sections of permanent metal decking had already been installed with each section measuring approximately 31-in by 81-in, with two sections installed only an opening of approximately 31-in by 81-in remained. As the workers approached the opening, they positioned themselves on opposite sides of the opening with the permanent metal decking held between them. The coworker crouched down to tie off to a loop of rebar that had been placed into the concrete. After he tied off, he held the metal decking with both hands, and suddenly felt it pull away from him. As he looked up, he saw the Employee #1 fall through the opening that was to be covered. The Employee #1 fell approximately 50-ft. and was killed.
