Employee injured when trips and falls; dies later from compl

At 5:20 a.m. on January 31, 2012, Employee #1 had just conducted a fire drill and recorded the information pertaining to the fire drill. Employee #1 was standing at the copy/fax machine in the nurses station making a copy of the fire drill report. A coworker had taken linen out of a patient's room and put the bag of linens by the nurses station for a minute to get something. Employee #1 was standing next to the copy/fax machine at the nurses staion and had just finished with the copy machine. He turned and didn't see the bag of linens. Both feet got tangled within the bag of linens and Employee #1 fell down and landed on his shoulder. Employee #1 died on March 17, 2012, of complications from right upper extremity injury.
