Investigation Summary
At approximately 3:00 p.m. on June 30, 2011, an employee was trying to locate the switch for the washer machine # 22 (Wascator, Type W-124, S/N 834/12273) in order to turn the machine off because it was malfunctioning. He attempted to locate the switch behind the machine with his hands. At that point, the employee received an electrical shock (220 Volt, 3-Phase). The employee died due to multisystem organ failure and delayed complications of the electrocution.
