Employee is run over by skid steer loader and killed

At 11:45 a.m. on August 15, 2011, Employee #1 was standing in back of a Woodsman 2112 trailer mounted chipper/shredder while the Employee #2 was operating a Bobcat S250 skid steer loader with a 'clam-shell' like attachment on top of the bucket. Employee #2 was using the Bobcat to remove logs from the site (a residential yard) where a tree was being topped out. On the second or third trip up into the yard, the Employee #2 picked up logs in the bucket, looked back to the right and left, then proceeded to back out of the yard where he was going to turn and dump logs into the GMC TopKick dump truck. While backing up, the driver ran over Employee #1, who was positioned at the back of the chipper. Employee #1 was killed. The Bobcat S250 skid steer loader was not equipped with a back up alarm as standard equipment; although it was an option at that time.
